  • Nurses do care plans regularly for patients 75 years and older, the same should be linked with health assessments
  • Front-of-house clinic staff should articulate the value of health assessments, as they often notice first signs of dementia in the elderly
  • Patient audits should be conducted to identify improvements needed in health assessments of the elderly
